These are the standard Frederick Ski Club forms and documents.

Choose the desired format from the list in parentheses after the name.
PDF – requires the Adobe Acrobat^® Reader. Open/download, print, and fill out.
PDF fill-in – same as above, but allows keyboard entry of data before printing.
RTF – is viewable by Internet Explorer^®, Microsoft Word^®, and other word-processing programs.

NOTE: you must be an FSC member to go on club overnight trips and other activities deemed risky

Trip Evaluation ( PDF, RTF) - suggested/encouraged for each adult trip participant

Trip Leader ( PDF) - forms required to lead FSC trips,

Check Request(PDF, RTF) - generally for trip/event leaders only

Club By-Laws(PDF)- the guidelines/rules/procedures for operation of the club

The determination of 'risky' is decided by the Frederick Ski Club officers, Board of Directors, and/or legal counsel.
